 For those with insurance:

  • Maryland Medical Assistance or Medicaid MCO

    • United Healthcare Community Plan

    • MedStar Family Choice

    • Maryland Physicians Care

    • Amerigroup

    • Aetna Better Health

    • Kaiser Permanente (Dental Only)

    • Johns Hopkins’ Priority Partners

    • CareFirst Community Plan

    • MD Healthy Smiles

  • Medicare (Coverage will vary for dental)

For those without insurance:

  • Proof of household income, including

    • 1040 or 1099 Taxes for the current year

    • 1 consecutive month of current pay stubs

    • Social Security Income letter for the current year

    • Food Stamp/TCA letter

    • 2 consecutive current bank statements

All patients will be asked to bring a current form of photo identification.
